Dr. Kasprzyk is Level I certified to see worker’s comp patients. This means that even if time has been missed from work, she is able to treat the patient. There are no co-pays or deductibles for work comp cases. All therapy and treatment offered in our office is available for injured workers. If your employer allows you to choose your provider then please bring your claim number, adjuster’s name and phone number to your first appointment.
If you already have an authorized treating provider who you were required to see by your employer, then you will need a written referral from that provider that it is okay for chiropractic care at our office as we are not part of the worker’s comp managed care organizations.
