When you schedule your consultation, our answering service will ask for your insurance information. If you have benefits that you would like verified, they will need the name of your insurance company, your ID#, your birth date and the customer service phone number.
Consultation start times are available between 8-10:30 am on Mondays and Fridays and between 1:30-4:30 pm on Tuesday, Wednesday, Thursdays.
Your benefits will be checked prior to your consultation so we may review your coverage during your appointment.
Our scheduling service will not be able to handle questions other than giving you appointment times and office hours.
But we would really like the opportunity to talk to you if you have any concerns that you need handled prior to scheduling.
